An investigative reporter just asked a key expert what she felt may have gone wrong at the level of the board of directors. Her answer was simply: Where was the Science? Where was the Engineering? She went on to point out that the issue at hand was a scientific one, at an engineering level, and that nobody at board level had any requisite expert level background.
In the software industry, which has regular, spectacular failures, this is a question that should be asked more often. It comes down to having a passion for the fundamentals of an industry, once a board starts to only looks at financials, that attitude will trickle down, with dire consequences from time to time.
